Complications during labor and delivery can result in severe birth injuries in a newborn. Oxygen deprivation occurs when the umbilical cord is compressed or twisted in the birth process. Mechanical trauma, which may occur when the baby assumes an unusual position at the time of birth (buttocks rather than head first, for example) or when the baby is too large to pass through the birth canal easily.

Obstetrical Brachial Plexus injuries happen during birth are known as (OBPI) and statistically occur in 2-5 out of 1000 births. These birth injuries occur as a result of a birthing emergency called shoulder dystocia. During the birth, the nerves can be stretched, torn, or avulsed (pulled out of the spinal column).

Symptoms of Brachial Plexus Palsy

Symptoms may include a limp or paralyzed arm, lack of muscle control in the arm, hand, or wrist and lack of feeling or sensation in the arm or hand. There are four types of brachial plexus injuries:

  • Aulsion, the most severe type, in which the nerve is torn from the spine;

  • Rupture, in which the nerve is torn but not at the spinal attachment.

  • Neuroma, in which the nerve has tried to heal itself but scar tissue has grown around the injury, putting pressure on the injured nerve and preventing the nerve from conducting signals to the muscles.

  • Neuropraxia in which the nerve has been damaged but not torn.Inflicted head trauma is a common cause of death or injury in babies at birth.

Brachial plexus palsy occurs by injury to the brachial plexus by force or traction on the shoulder during delivery. Large infants are usually affected by brachial plexus palsy resulting from force of pulling them from the birth canal. When the shoulder is forcibly pressed downward, the brachial plexus can be stretched or injured.

The delivery method usually correlates with the type of brachial injury. Infants with upper brachial plexus injury (Erb's palsy) are more likely born by breech delivery, and those with more extensive injury are more likely born headfirst. However, brachial plexus palsy can occur even with cesarean section.
